Please be advised that Form 1095-C for the 2025 tax year is available upon request.

To request a copy of your Form 1095-C, please contact Eastridge Workforce Solutions at benefits@eastridge.com.

Requested forms will be provided within thirty (30) days of the request. This notice will remain available through October 15, 2026.

California Employees:
If you are a California employee, Form 1095-C will be mailed to you automatically. No request is required.
